GDPR Statement ​

​At various times throughout your interaction with myself at Massage and Move you will be asked a number of personal questions which will be documented for treatment purposes only. 

Below informs you what information Massage and Move requests and why, what information is stored and how it is processed. 

Appointment Booking 

All appointments are booked directly with myself, this is done through social media platforms or via phone/text, there is no current online booking system.  If you are a new patient to Massage and Move, when the booking is being completed personal details will be asked for at this stage such as address and contact telephone number. 

A new patients address is asked for at this stage of the booking as Massage and Move provide a mobile sports massage, the contact number is also asked for, if the new patient has booked through a social media platform to make it more accessible to contact the patient. 

Initial Consultation 

In the initial consultation, I require your full name, address and date of birth, so that I am able to identify your medical records correctly and to be able to identify your documents appropriately if there is to be any necessary correspondence with 3rd parties via your authorisation. 

If any of your circumstances should change such as current medical issues, change of name or address then please ensure that you inform Massage and Move so that your records can be updated. 

Your Medical Records

Massage and Move is legally required to record any essential clinical and treatment information, that is obtained from you during your initial consultation and subsequent appointments.  This is to enable myself to make accurate understanding of the presenting problem(s) and to provide an appropriate treatment and management plan.  These records are paper based, and are stored within a locked filing cabinet. 

Your medical records, and any correspondence such as referral letters and reports based on their contents, are not shared with or passed onto any 3rd party, with the exception of: other healthcare professionals and only with your consent. 

Statutory minimum storage times for medical records and associated correspondence are 8 years after the date of the last appointment. 

Data Processing 

Massage and Move maintains a paper copy from the initial consultation which has the above details stored on as well as information regarding your presenting injury, findings and subsequent treatments. Paper documentation is stored within a secure and locked filing cabinet, which is only accessible to myself.
